I have encountered a problem when I tried to get acess to Sourvey Solution Headquarters. What is the detail of username and password?

Can you provide more information?
Is it a new installation? What happened before? How many people have access to this account? Any chance that somebody changed the password? Have you run heath-check util?

this is not a problem, but a normal use case. Access to data is only available to the persons, who have been authenticated by the server administrator. Contact your server administrator to request an account.
Cannot help any further. Your screenshot conveniently cuts the address of the server.
