If you’re implementing this in CAPI, then I would suggest making each row of the table its own question (e.g., p7_1, p7_2, etc.). Since the questions share the same answer options, consider using reusable categories.
If you’re implementing this on CAWI, have a look at the tablular presentation of rosters.

If I’m not mistaken, the CAWI mode is active when the interview viewed in the web browser. This question will either not appear or appear differently in the Interviewer app.
To be honest, I’ve not used CAWI mode. Others are likely better informed than I.
