The error “Entry was not found in questionnaire, please select another.” occurs when you are trying to navigate to an item which no longer exists in the questionnaire (just like the message says).
This can happen if you open the same questionnaire e.g. in two different browsers and delete a question in one of them, then try to select it in another. Same will occur if you do this from a different device or if your collaborator moves/deletes it while you are working.
What should you do? Refreshing the page will actualize the changes done elsewhere and remove from your view the elements already deleted by others.
