Bug in CAWI even if form compiles correctly in designer

Hi Colleagues:
I am having a problem getting the weblink to work even if the questionnaire on the designer is compiling and the questionnaire is working in the tester online.

I generated a weblink with a password and when I attempt to login, I get this screen shot:

Can you tell me what the problem might be?

Make sure to assign to an interviewer, not a supervisor.

Thanks! the interviewer follows up on the interview if there are issues when the CAWI user completes it. Totally missed that.
